https://journals.unam.mx/index.php/atencion_familiar/article/view/91024

Objective: To identify factors associated with polypharmacy in elderly patients. Methods: analytical cross-sectional study carried out between March and April 2023, 440 adults >60 years of age from a family medicine unit in Acapulco, Guerrero, Mexico, participated. Descriptive statistics were performed, and frequencies and percentages were obtained, as well as measures of central tendency and dispersion; contingency tables were used in the bivariate analysis; a p value <0.05, and 95% ci were considered statistically significant. Results: 63.6% of the adults presented polypharmacy, and 7% excessive polypharmacy; a mean age of 72 years was observed, with a higher prevalence in women (55.4%), the most relevant predisposing factors were chronic diseases such as diabetes (p<.001, or 9.26, 95% ci 5.24-16.36), and arterial hypertension (p<.001, or 5.06, 95% ci 3.24-7.90). Conclusion: six out of ten elderly people presented polypharmacy, a significant association with diabetes, arterial hypertension, and dyslipidemia. Identifying the vulnerable population is key to manage the control of the disease and guarantee a rational prescription to minimize risks.
Guillermo M. https://journals.unam.mx/index.php/atencion_familiar/article/view/91026

Objective: To analyze the main characteristics of the most cited articles published in the medical journal Atención Familiar from 2025 to 2023. Methods: A bibliometric analysis was conducted, selecting all article cited ≥10 times (i10-index). For each article, the topic, type of article, study design, and citation count were identified using Google Scholar, including the number of citations in Spanish and the type of citing documents (e.g., journals, theses, or books). Results: A total of 74 articles met the selection criteria. Of these, 46% addressed topics related to diabetes mellitus, obstetrics and gynecology, and geriatrics, while the remaining articles covered other subjects. Regarding article types, 66 (89.2%) were original research articles, of which 57 (86.4%) were cross-sectional studies. The articles accumulated 1,910 citations, with 1,735 (90.8%) appearing in documents in Spanish, while the remaining citations were in other languages. A total of 1,169 citations (61.2%) appeared in graduate theses, while 741 (38.8%) were cited in journals. Conclusions: the most cited articles in Atención Familiar focused on primary care topics such as diabetes mellitus, obstetrics and gynecology, and geriatrics. Original studies, primarily cross-sectional, predominated. Most citations were in Spanish, with a strong presence in academic theses. These findings reflect the journal’s influence on academic training and scientific production in family medicine.
Rocío Dávila-MendozaGeovani López-Ortiz

https://journals.unam.mx/index.php/atencion_familiar/article/view/91027

Objective: To evaluate the effect of a psychoeducational intervention on the level of mental health knowledge of undergraduate medical interns, and to know their opinion about this activity. Methods: randomized controlled trial with mixed approach. Seventy undergraduate medical interns participated, divided into two groups: intervention and control. The first group participated in theoretical sessions and cognitive-behavioral strategies. The level of knowledge about mental health before and after the intervention was evaluated, and the opinion of those who were intervened was obtained by means of open-ended questions. Means and comparison of means, Student’s t-test and qualitative analysis of the open questions were obtained. Results: in the first evaluation the mean score of the intervention group was 4.6, and that of the control group was 5.0 (mean difference: -0.48, p= 0.09), after the educational intervention the mean of the intervention group was 8.17, and that of the control group was 5.10 (mean difference: 3.077, p= 0.00). In the qualitative analysis it was identified that the students improved important aspects of their mental health and consider this type of strategies necessary during their education. Conclusion: the students who participated in the psychoeducational intervention showed improvement in the level of knowledge about mental health issues and improved the perception of their emotional state.
Baltazar Joanico-MoralesYanet V. https://journals.unam.mx/index.php/atencion_familiar/article/view/91029

Background: Validity and reliability are frequently misinterpreted and improperly applied in various types of studies. Therefore, it is essential for healthcare professionals to use appropriate measurement instruments for assessing subjective attributes and complex dimensions. This ensures accurate interpretation of evidence and improves medical care. Objective: To propose useful criteria for selecting appropriate measurement instruments in the health field to evaluate variables of interest. Methods: A literature review was conducted. Using search algorithms in various databases, narrative and systematic reviews published between 2000 and 2023 that addressed measurement instruments were selected. Results: Of the 351 articles initially identified, 62 met the selection criteria. The key criteria considered for selecting measurement instruments included cultural adaptation, reliability, validity of theoretical constructs, validity of empirical constructs (criterion and construct), and responsiveness. The most relevant aspects of these criteria were defined, classified, and analyzed in terms of their usefulness. Conclusions: Selecting measurement instruments for variables that cannot be directly measured requires a thorough search and evaluation of the properties described in this article. These criteria provide a more objective approach to selecting measurement tools, enabling more precise assessments of the phenomena under study from both qualitative and quantitative perspectives.
Francisco Javier Fulvio Gómez-ClavelinaGeovani López-Ortiz

https://journals.unam.mx/index.php/atencion_familiar/article/view/91030

Introduction: Compartment syndrome is a pathology secondary to increased tissue pressure within an osteofascial compartment. Objective: To review the recent literature on the diagnosis and treatment of compartment syndrome to provide an updated view of this condition. Methods: A search of the Web of Science and PubMed databases was performed, using literature from 2019 to 2024. The search terms used were: compartment syndrome, diagnosis, treatment, extremities. A total of 179 articles were found. The selection criteria were: original articles, with available references, from refereed publications, less than five years old, updated articles on prevention, diagnosis and treatment, that had a relevant contribution to this research. After submitting these texts to the selection criteria, 28 articles were obtained. Results: Within its physiopathology, arteriolar vasodilation and collapse of the venule system play an essential role, which conditions an increase in vascular permeability, ending in the extravasation of liquid to the interstitial space, causing edema and a decrease in the rate of tissue perfusion. Its diagnosis is mainly clinical, by means of a clinical history and an adequate physical examination. The definitive treatment, once this pathology is established, is fasciotomy. Conclusions: Mortality in a patient with compartment syndrome can reach 47%, so a timely and efficient diagnosis is essential.
Joaquín Palmero-PicazoJareth Lassard-Rosenthal
